whom we have a Producer Agreement. We may provide quotations to producers without
agreements, but policies cannot be bound until there is an executed brokerage
agreement with J. H. Ferguson & Associates, LLC or the United National Group.

We will accept another company's application if there is sufficient information to
provide a quotation. We will later need our application completed if an account is bound.

Financial information, ideally a CPA audit, should be included with each
submission. We can download 10K's and other public information. Please
E-mail private placement memorandums and CPA audits where possible.

Our quotations are valid for 60 days. After that time period we may issue a
new quotation. However, we cannot guarantee that the premium or conditions will
be the same as the original premium quotation. Renewals must be bound on or prior
to the expiration date.
